I'm seriously out of ideas.
The idea of the monster as being a nanotech reation can now be ruled out..... nanobots don't recreate memories, and they're not exactly smart enough to gauge fear.
Though that being said.... it is said that a lot of the monster's abilities are tied to mental states.... so, this is the most I can think of:
The reason why the smoke monster is percieved as a cloud of smoke is, in very typical Cthulhu terms: it is what we can afford to percieve it as. Its true form may be something entirely else, but it takes on that form since it is the easiest on the mind, and the most perplexing to the survivors (perhaps it may be the same Cerebrus of legend, where it took the look of a three headed hellhound to scare off everybody)
This theory may also explain why it loves to take on the identity of past figures..... and appear off the island.
Essentially, smokie has less to do with the island then it has to do with the world. Yes, it protects the island, but it is not necessarily restrcted to the island.
